- Management of long-term conditions (e.g. diabetes, asthma, hypertension, arthritis, COPD)
- Referrals to specialist services
- Cervical Smears and Sexual Health Advice : Routine smear tests to help prevent cervical cancer
- Preventative Health Care & Health Checks: NHS Health Checks and lifestyle advice to reduce risk of illness
- Midwifery Services: Antenatal and postnatal care in collaboration with maternity teams
- Childhood Immunisations
- Immunisations (e.g. flu, Shingrix, Pneumococcal ..)
- Minor Surgery – Joint Injections: Treatment for joint pain and inflammation
- Smoking Cessation Support
- Prebooked and same-day appointments for routine and non-urgent medical needs
- Extended Hours Access : Appointments available outside normal working hours
- Phlebotomy Service: Blood sample collection for diagnostic testing
- Locally Commissioned Services(LES): e.g. Phlebotomy, Shared Care Services, Learning Disability Health Check, Weight Management referral, Care Home Support, Dementia Support
- Services Provided via Primary Care Network (PCN) : From time to time, additional services may be offered through our PCN collaboration. These may include:
Specialist Clinics such as mental health support, Physiotherapy and Musculoskeletal Services
Social Prescribing: Connecting patients with community-based support and activities
Health and Wellbeing Coaches and Mental Health Practitioner Clinic
Care Coordinators to support patients with complex needs
Vaccination Campaigns beyond routine immunisations
Enhanced Access Appointments across PCN member practices - National Screening Programme – Patient Advice
We encourage all eligible patients to participate in NHS national screening programmes. These are designed to detect early signs of illness and improve health outcomes:
Cervical Screening: For women aged 25–64
Breast Screening: For women aged 50–70
Bowel Cancer Screening: For adults aged 60–74
Abdominal Aortic Aneurysm (AAA) Screening: For men aged 65
